HOME MEDICINES REVIEWS (HMRs)
- A credentialed pharmacist will visit you in your own home, upon referral from your doctor, to review your medications, provide you with tailored information, and work with you and your doctor to create the best medication plan for you. HMRs are aimed at helping you better understand your medications and how to manage them, minimise harm from medicines, and make sure medicines are being used in the best way possible. HMRs are usually funded by the Department of Health and Aged Care, at no charge to the patient.
